Membership fees start as low as $167 per month or $2,000 annually.
You do not need a membership to be a patient of the practice. However, your care will be provided by our nurse practitioner if you choose non-membership status.
Dr. Hayes will supervise this care. To be seen by Dr. Hayes exclusively, you would need to sign up for membership.
We accept Anthem, Blue Cross Blue Shield, Cigna, Aetna, Humana, United HealthCare, Tricare, Medicare, PHCS, Multiplan, VA Community Care, Ambetter Core, Ambetter Select Plus and fee for service (credit card, cash, HSA/FSA card). Sorry, we do not accept personal checks.
Deductibles, co-insurance and copays are expected to be paid at the time of service.
Our doctor does not admit patients to the hospital. We use hospitalist or hospital doctors to manage our patients in the hospital. In this manner, Personal Concierge MD gives you more time with the doctor in the outpatient practice.
Our office has OSHA graded HEPA filters and UV light in our HVAC system to keep you safe from COVID-19, Influenza and other harmful bacteria. All staff and patients are required to wear masks, wash hands and practice social distancing.
No. Dr. Hayes is an internist which is an adult care doctor. If you desire concierge services for your children, Dr. Hayes can refer you to a concierge pediatrician.
